It is your right to:

    • To be greeted in a welcoming and caring manner

    • Have privacy and confidentiality

    • Be treated politely and with respect by all members of the team

    • See your health records (as far as the law permits)

    • Make a suggestion or complaint about our services

We welcome your comments to help us improve. If you want to make a complaint, please speak to the Practice Supervisor, who will explain the procedure to you, or contact the Patient Liaison Group.

Our commitment is to:

    • Answer the phone promptly

    • Tell you of any delay during the surgery

    • Answer any question so you understand all the information and advice we give you

    • Emphasise health promotion and prevent illness

It is your responsibility to:

    • Let us know if you change your name, address or phone number

    • Let us know if you cannot keep your appointment

    • Treat all our staff politely and with respect

    • Ask for a home visit only if it is really essential

    • Take responsibility for your own health and take advice on how to prevent ill health
